"Smoked" is the term used when the player runs out of health in Saints Row, Saints Row 2, Saints Row: The Third[1], Saints Row IV and Saints Row: Gat out of Hell.
Overview
There are a large variety of ways to "die" in the Saints Row series; from being run over, to being beaten up, or shot at, set on fire, caught in an explosion or falling from a substantial height. No matter how the player dies, they always die when their health meter becomes empty. Thus, being "smoked", as the game calls it, is basically a Game Over. However, this is usually little more than an inconvenience, as after a short loading screen, the player can learn from their mistakes and retry whatever they failed at.
If the player dies inside of a mission, they are given a choice to either retry the mission or quit it. If the player dies inside of an Activity, or outside of a mission, they respawn at the nearest hospital.
No weapons are removed when Smoked.
Saints Row
Getting Smoked advances the game time by 6 hours.
Stilwater appears to have a socialised health care system, in which a person's savings are used to determine the cost of treatment after being Smoked. The cost is 20%, so the poor pay a smaller amount than the rich, but receive the same level of care, which is basically how health care works in the majority of the world.
There are no mid-mission checkpoints, so the only options when dying during a mission are restart or quit.
There are 4 hospitals where The Protagonist may be taken after being Smoked.

Saints Row 2
Being killed in Saints Row 2 works largely like the previous game; with the addition of optional checkpoints for missions.
Stilwater has overhauled their healthcare so there is now a flat fee of $500 for treatment.
There are 4 hospitals where The Protagonist may be taken after being Smoked.

- Medical Center - Stilwater Nuclear
- Prison Infirmary - Stilwater Penitentiary
There's also St. Matthew's Hospital in Athos Bay but The Protagonist is never taken there after being Smoked.
Saints Row: The Third
When dying in Saints Row: The Third, the "You're Smoked!" message doesn't appear on the screen, it simply turns red. When dying for the first time outside of a mission, the game still refers to this as "You got smoked!".

Similar to Saints Row: The Third, there's no "You're Smoked!" message appearing, and the screen doesn't turn red. Instead, there are glitch effects shown on-screen. When dying for the first time outside of a mission, the game refers to this as "You died!".
The Protagonist respawns near the same hospitals as in Saints Row: The Third, although at different locations.

Trivia
- Unlike Saints Row, when dying in Saints Row 2, the screen turns red when the player dies.
- There is a glitch in both Saints Row 2 and Saints Row: The Third, where it is possible to successfully complete a mission or activity without being killed but respawn outside a hospital, minus $500, but without the death being registered in the statistics. This commonly happens with the mission "Showdown".
- Pausing and unpausing the game repeatedly while the "You're Smoked" message is on screen can cause the game to glitch and stay on that screen.
- If Smoked during a Bonus Mission phone call in Saints Row: The Third, the "Mission Failed" screen is shown, and the option to restart the mission is given. If the mission is exited, the Cellphone rings again.
- In Saints Row IV and Gat out of Hell, the player doesn't die if the vehicle they are inside explodes.
- In Saints Row: The Third it is possible for one to team kill in one shot while in a Garage menu and their teammate falls through the level. In addition there are 2 character load glitches outside of missions, the infinite ammo duplication glitch which requires 2 characters the first one a level 50 with infinite ammo loaded and then another character and the instant revive glitch, which requires you to get downed and reload your character, these 3 glitches can only be done in Co-op, as the load glitches require you to be player 2 and the team kill glitch requires you to kill a Co-op teammate.
- In Saints Row: The Third you cannot get killed by a single vehicle explosion if you don't bail.
